[Forum] Moderated threads not being approved

When I try to moderate a thread, I select "approve" then click "save" at the bottom and it says something like "Moderated threads successfully" - then refreshes back to the threads needing moderation and they are still there! This happens regardless of approve or delete - they just get "ignored" and put back into the queue.

SOME of them (normally at the very bottom of the queue) can be moderated successfully but I almost always have to go into the actual forum and spot the post to approve or delete it or not.

Any ideas?

Did you tried clearing your browser cache and deleting the cookies?

I have cache permanently disabled and my cookies have been deleted multiple times - it's been ongoing for a couple weeks since I turned on thread moderation.
