I've been able to replicate this on my forum, as well as two other forums (including vbulletin.com), using the latest Firefox (15.0.1) - both on my main computer, and on a computer with a clean install/clean firefox install.
I don't know if there was a recent update in Firefox or something, but this seemed to just start happening for me.
My forum runs on 4.1.8, however this error is now present in 4.2.0 as well - including here on vbulletin.com.
It's possible it has something to do with ajax - I have randomly been running into issues when doing quick replies where the ajax post fails and it does a full refresh of the page after posting (thus resulting in a "Are you sure you want to leave this page" message).
This started happening around the same time this image issue popped up as well, thus why I think it may be something ajax-related, as ckeditor uses ajax for inline image uploading as well.
You can easily replicate this issue.
1.) Start a new thread, or do a quick reply to an existing thread.
2.) Insert an image via the Insert Image button (it will then appear in the editor properly). Press enter twice (to create a break between the two images).
3.) Insert another image via the Insert Image button. The image will not appear in the editor window, however it will attach itself to the post.
If you try to do this same thing in Internet Explorer or Chrome, it will work properly - both images are inserted inline.
Only by inserting a break-line (pressing enter) will cause this function to break. I'm not sure why.
Again, with the timing of these two issues, and the fact that they both rely on ajax, leads me to believe there was some sort of modification to firefox that changes the way ajax operates.
Any insight or help with this issue would help appreciated!

Just an FYI, I just installed Firefox 14.0.1 and the functionality issues listed above have disappeared. Perhaps I should be posting this in a firefox forum instead of here.
