[Forum] vBulletin Database Error! Error Number:1062 - Duplicate entry 'xxxx' for key 1

Hi all! Dear professionals I need your help.

After upgrade from 3.8 to 4.1.1, currently I have an issue: nobody can register.
After registration I'm getting a message: "Database error" and receive an e-mail with this detailed info:

Hi,

I would like to know the fix to that database error message.

In my case:

INSERT INTO tagsearch (tagid, dateline)
VALUES (435, 1308850132);

MySQL Error : Duplicate entry '435' for key 1
Error Number : 1062

By the way, I found this entry on your manual:

https://www.vbulletin.com/docs/html/mysql_duplicatekey

but there is no mention to table tagsearch in there.

And also, an error I experienced when upgrading to 4.x:

ALTER TABLE usergroupleader ADD index ugl (userid, usergroupid);

MySQL Error : Duplicate key name 'ugl'
Error Number : 1061
