[CMS] CMS allowing the guests to see pictures

What have you got set for in the Can Download Attachments column for the Unregistered/Not Logged in usergroup in AdminCP > vBulletin CMS > Permissions?

Guest can't download attachments, so I must conclude that to display images with the img bbcode guests must be able to download attachments present.
I'm sorry but download a file and see the images are two completely different actions, was not a good choice in my opinion bind the two actions
