It does support it. You can test it right here. Look at the bottom of this page, there's a bottom menu bar. From the drop-down on the left, where it reads "-- Englisch(US)", choose "-- Right-to-Left Test". Voila.

All existing translated phrases and language packs will still work with vB4. However there are new phrases in vB 4.0 that you will need to update and provide translations for.
