      Now I know it can not be a browser issue, as it show in both IE 8 and FF 3.6.11 (It is not cache or any of that, cause I wipe my drive earlier this month.) It seems to be just sporadic

        If I had to guess, we still do moderately manual style upgrades, we might have imported a new settings file that had an old version number.
