link to specific forum with specials custom fields

Hello:
I want give to my customers an speficic link, that only they will have to the forum where they could insert testimonials...i need that when they go to this forums ( as guests)...have to fill some custom fields...domain registered with us, customer since, rate de service, etc...is it possible to do this into my vbulletin admin area ?
