Some users seem to bypass User Moderation

Most of my new users have to verify their email address, and then go into the User Moderation queue to be accepted or deleted. Others, however, seem to bypass moderation - they don't appear in the User Moderation queue, but appear immediately as users - albeit with no assignment to user groups at that stage.
No logic to who does and doesn't get straight in. Anyone know what's happening here, and how I can stop it?

This does not happens with the original vB code. Have you installed any add-ons or plugins?

I just had this happen to me too for the first time.

Very weird. Seems as though Vbulletin is a sensative program, edit just a tad too much and something breaks.
