    you need

    1. telnet/shell access
    2. need to run this command in shell

    mysql -u mysqlusername -p emptynewdatabasename < /path/to/backupfile.sql

if you don't have shell/telnet access you need to ask your host to import your sql backup file into an empty mysql database for you
