Double Posting in forums when using quick reply option

We are getting double posts when a user uses the quick reply option. Is there any way to troubleshoot this issue?

I have also noticed that sometimes after posting using quick reply, my post still remains in the new quick reply after my post has already been posted. If one does not notice this, one can post second time I guess.

The fact that it happens sometimes makes me wonder what is going on. I am not sure this is our issue but it could be the reason.
