    Most of the vB sites I've seen have template modifications or add-ons that would break and cause problems if there was a script that deleted all non-vB default install files by default.

    It's a good idea to keep a manual list of which files are from vBulletin and which are from 3rd party modifications as well as a separate list of which templates have been manually edited.

    Sometimes the Suspect File Versions tool will give a false flag.
