Database error while trying to change subscription dates

Just did the upgrade to 5.1.9 from 4.23. Looks like everything is working except we cannot change the subscription dates for our subscribers. We get the following database error:

    Database error in vBulletin 5.1.9:

    Invalid SQL:
    INSERT INTO forum2access (`userid`,`forumid`,`accessmask`) VALUES (5195,6,1),(5195,32,1),(5195,27,1),(5195,31,1),(5195,5,1),(5195,29,1),(5195,30,1),(5195,3, 1),(5195,13,1),(5195,7,1),(5195,8,1),(5195,28,1),(5195,25,1),(5195,21,1),(5195,24,1),(5195 ,34,1),(5195,9,1),(5195,11,1),(5195,10,1),(5195,12,1),(5195,14,1),(5195,4,1),(5195,26,1),( 5195,15,1),(5195,17,1),(5195,16,1),(5195,22,1),(5195,19,1),(5195,18,1),(5195,20,1)
    /**access**/;

MySQL Error : Duplicate entry '5195-6' for key 'PRIMARY'
    Error Number : 1062

Can someone PLEASE help me, we have people that have paid and we can not make the changes to their subscription.

Can you be more specific on where you're making these changes at?

admin panel> users>username> and then make the changes in the user manager area.

We make the change in the Paid Subscriptions area by changing the dates

Hmmm. I don't have that table in vBulletin 5.1.9. Nor do I see it in a vBulletin 4.2.3 database. Do you have custom coding for this? This coding seems to need to be updated.

No special coding, out of the box 5.1.9 that was just downloaded last week. We go into the admin panel, go to users, do a search for the user we want to find, then click on that user. Inside the user manager for that person there are 2 ways to access their paid subscription area. One is to use the quick user links and choose "add paid subscription" or the other is on the right side of the screen we have a "paid Subscriptions" under the usergroup options. Either way we can change their subscription dates. We have done this for a long time with 4.???, but after the upgrade we get the above error. Please help, we have people that have paid but can not access areas of our site because of this.

          Thank you

This is what the site says, the database error is email to me:

"That action could not be completed. Please try again, and if this occurs again please contact the system administrator and tell them how you got this message."

Did you have any addons on 4.2.3?
That dtatabase table is not a default vBulletin table.

When you upgraded, did you make sure you moved ALL old vB4 files and folder s out of the forum folder? (With the exception of attachment /avatar / sigpic folders).

Yes, fresh upload to a new directory, and even a new domain. The only folder from 4 was the Attachments, everything else was from the upload folder in 5.1.9
