    Trouble with attachments/uploading images 5.1.2

I was hoping I could get some help or suggestions. The attachments functionality or uploading images seems to function but a broken link is the only thing that is ever displayed. I have tried every setting in the admincp to get this to work but just can't seem to get there. Some info:

    vB version 5.1.2
    MySQL version 5.5.30
    PHP versioin 5.4.13
    No addons installed
    Issue occurs in default style
    Issue occurs using English
    Occurs in all browser versions

To recreate the issue I just use the attachment button or the insert image button and try to upload the picture. I have gone through the diagnostics and tested the upload and it does not report any errors. I have tried both attachments in the database and attachments in the file system. The upload in the diagnostics reports:
    Pertinent PHP Settings
    file_uploads: On
    open_basedir: /var/www/vhosts/site.com/:/tmp/
    safe_mode: Off
    upload_tmp_dir: None
    upload_max_filesize: 128.00 MB
    Help
    Results
No errors occurred while opening the uploaded file for reading. You should be able to turn off the "Upload In Safe Mode?" option without any adverse effects.
When I upload the image and I look at the link to the image it shows:

    www . site . com/forum/filedata/fetch?id=2335&d=1405001843

(without spaces, just trying to show full link)

When I upload using attachments it shows something similar. In all cases the attached image place holder shows inline in the post but just as a broken link. Thanks for any help.

Another piece of information, if there is a direct link to the image it has no problem displaying (i.e. http://www.site.com/image.jpg) but anytime its a reference to fildata/fetch it will not display. This is true for any image on the site including header logos, etc.

I would suggest raising a support ticket, we're going to need to look at your setup.

We'll need both admincp and ftp access.

Can you give us information about this problem Mark ? Cause, just once Zachery solved this problem in 5.1.1 but never say how he did this. Waiting information about this.

What do you have set for AdminCP > Settings > Options > Server Settings > Safe Mode Enabled and Safe Mode Temporary Directory? And how about AdminCP > Settings > Options > Image Settings > Image Processing Library? And, is there anything in your error_logs (if you don't know where they are, ask your host) at the time you try your upload?

Please don't PM or VM me for support - I only help out in the threads.

There is Server Settings in first screenshot and Image Settings in second screenshot, there is no error log. It looks like as broken link when i upload a image or picture like in third screenshot. Also there is Diagnostics Result in fourth photo.
