[Forum] Database Error on adding New User manually...

I have used this in the past with no errors, but this morning I am attempting to manually add a New User; but every save of details throws the following error....Anybody have any suggestions? The Forum is currently closed to automatic registration.

Into the new member profile I am adding

Username
Password
email address
and setting primary membership status to "waiting email confirmation" - alternative of "member" also didn't work.

Yes this is related to an add-on since it seems to be referencing a thread, which wouldn't normally happen on a default installation when creating a user.
