There're a lot of factors that can cause this issue, I'd suggest to disable vBSEO and go back to the barebonds vB URL scheme (with redirections of course), keep your forum up to date and of course disable any outdated plugins.

You may upgrade to vB5 too if you're comfortable with it.

You went through every point in this topic: https://www.vbulletin.com/forum/foru...ring-your-site

Did you review every line of code for every plugin? Did you make sure that all template edits are proper?

This exploit is usually done in an edit of the template_parse field of the template table. You won't see this change in the AdminCP. It could be a plugin. It could be changes to the javascript. It could be an Ad on your site. Access is usually through the database is done through a rogue plugin or a file that shouldn't exist in a vBulletin installation. Could be vBSEO or any other plugin that allowed access in the first place. Could be a security issue that was patched but you were exploited before you applied the patch. The exploit could have happened years ago and you're just now seeing it or it was just activated. Heck if you left the install directory online for a number of years, the person doing the exploit could have an admin user on your site.

We know many solutions to the problem. However, without access to your server and database, we will never know the solution that fits your particular instance of the problem.

vBulletin 4 comes with tech support. If it is a vBulletin 4 license, then submit a ticket. The sarcasm won't help though.
