Reply toolbar greyed out for all users after upgrade

I upgraded to 4.2.2 and now the reply toolbar is greyed out. I have tried disabling all plugins (no luck), I have made sure that my style settings for mobile and older browsers are set to none (no luck). The default style has the same issue. Here is what all users see. Everyone is set to Standard or Advanced editor. I tried creating a new style with no parent (no luck). I tried disabling hooks (no luck). Quick reply has the same issue. If a user wants to reply to a message and not use the quick reply, but the advance reply instead, they have to click the "Reply to thread" button 3 to 4 times before it will direct them to the advanced reply page.:

Is icons.png located at clientscript\ckeditor\skins\kama?
