4. click request username/password now

So you can not log in to the forum with the userinfo that is emailed to you when you through the forgot password link?

Can you access PhpMyAdmin or run quires?
If yes run those queries
run them one by one,

 TRUNCATE TABLE session;
 TRUNCATE TABLE strikes;

then reset your password, and login to your forums
