[Forum] Custom Profile Field, listing user's kids & calculating ages?

i would like to have profile fields for my Users to list their kids and the kids' birth dates. In the end I would like it displayed with the kids age that updates automatically based on the birthdate that was entered.

since this is a forum for parents of twin and triplets and such the categories would need to be something like

pull downs [singleton, twin, triplet, quad]
then it give you the right number of fields to fill in data on each kid:

pull down [babyA,B,C,D], pull down [boy/girl], fill in [name], pull down [hide kids name/ use initials/ show name in profile] pull down [birthdate]

and they could just keep adding more kids till they were done.
i would also ilk them to be able to put a future date to denote pregnancies and it show a actually month and year date, rather than the calculated age.

i then want in the user info block in a post to show something like this
