Advanced post editor icons not showing up

I had exactly the same issue but with vBulletin 5.x, like on first screenshot below:
Then, I looked at the page source (second screenshot) and I found that the icon file was not in place (I don't know why, its not there even in the source tar...)
So, I copied it from another location ind the is fixed!
I hope this will help someone.

No vB4 issues will ever impact vB5 as the products are completely different.

The issue you mention is a bug in 5.4.4 and is fixed in the next release:
