how to prevent what happend to this forum ? (hacked)

Apparently this forum was hacked earlier this week, therefore the passwords where reset.

How did the hackers gain access to the database ? Was VB hacked or was some other server software hacked ?

how do we protect our own forums ?

I browsed these forums but found no info regarding this issue.
