I've noticed in the asset manager that all of my old attachments still exist, one's that I've removed from posts and some that I've never decided to attach to posts. Aren't these supposed to be deleted automatically?

I think that might be because you've used them, so even if you remove 'em from posts, they've already been utilized. Don't quote me on that, though.

Then there should be a way to remove them. Unused attachments don't appear in your UserCP or AdminCPComment

My server runs on PHP 5.4. From my understanding vB4 can have some issues on PHP 5.4. Could this be the cause?Comment

What VB 4.x version are you using? Officially there was a bug fixed in 4.1.10 - http://tracker.vbulletin.com/browse/VBIV-6994
If you're having the issue in a later version it could probably be re-opened, I don't see any other open bugs on this issue.
