Editor toolbar font colors doesn't function correctly

We've been having a problem with the editor bar function when you create a post. It's been broken for several years, but we were on an old version of vBulletin. I thought it might get fixed when we updated. Yesterday, we finally updated to 3.8.11, but the problem remains.

The problem: in the message formatting toolbar, everything works fine except the dropdown menu that should allow you to select the color of your words. When you click on it, instead of getting a dropdown menu with the standard colors, you get one tiny box with white and no other options (see photo of our editor bar vs. one that's working on another vB 3.8.x forum).

This function worked when we started back in 2005 but it's been broke for a while.

I looked at the code in the editor_toolbar_on template, but I didn't see anything wrong. I compared the code on a forum where it's working but the code looks exactly the same.

What is your php version?

3.8.11 requires a minimum of php 5.6.

We're on PHP 5.6 -- we just upgraded so we could install vB 3.8.11. To be clear, the problem was occurring before we upgraded our php version.

This doesn't happen on a default installation.

Do you have any third party add-ons?

We've done some customization over the years (I'm not the only guy who has touched this). The two that might be most relevant are the strike-through and YouTube buttons in the next row in the same editing toolbar group. You can see them in the image. I didn't add these, but if I knew where to look I could remove them and see if that fixes the problem.

Any hints as to where I should be looking? I don't mind doing the work, I just don't know where I should be looking.

They could be added as a plugin, they could be file/template edits - it's hard to say. If you check the Plugin Manager first, is there anything relating to them there?

I tried disabling ALL our plugins, but that didn't fix the problem.
