Does anyone have any good pointers for how to clean this up? I have already attempted to replace the site files - but that didn't seem to help. Any other ideas?

The issue could be on the hosting side. Contact your host and have them detect the entry point of the hack.

Chances are one of your plugins were hacked. Check the global_start plugins and see. Make sure you scroll to the bottom of each plugin. Not sure if you are using MySQL 5.5 or higher, but if so, you could use this to protect yourself from future hacks:
