Posts into moderation

What would cause random posts to go into the moderation queue? New members, long time members, etc. Totally random. One post from a single member is ok and then next one from the same member goes into moderation.

When did this start to happen first? Did you do any modification to your forum prior to noticing the issue?

Not exactly sure when it started happening but I haven't done any mods at all the the forum in probably more than a year.

I have a mod that will put posts into moderation if they contain an URL or spam key word as an anti-spam defense... but you'd probably know if you had something like that.
