Double posting

I have double posting issue which is happening in all of the browsers. This issue does not happen always, most often this happens with the moderators
We have PHP Version 5.4.4-14+deb7u14
vBulletin 4.2.2 Patch Level 1

Does the same thing happen on a unmodified default style with all mods disabled?

You need to deploy 4.2.2 Patch Level 2 straight away.

I would try upgrading to the latest Beta if disabling mods doesn't solve the issue.
