Going from 3.03 to 3.8... Do I have to update or can I install new?

As long as I have my dbase backed up, can I go from 3.03 to 3.8 using either method?

Is one better than the other, aside from ease? Part of me would like to start "fresh" with 3.8 but if it's a PITA to get my old posts/users/etc back correctly then I guess I'll upgrade.

You can upgrade directly. You will need to use the 3.8.0 config.php file, and of course you should make a backup of your database first. Please follow the upgrade documentation here:
