There are two setting for posts/threads to end up in the moderation que.
one is per forum and the other is per usergroup. you can set a forum up where all posts in that forum need moderated or you can set a usergroup permission where posts need moderation. you will need to check each of these per user and per forum as far as I know. There may be a faster way, but that is how I did it.
Q2, usergroup settings. registered, guest, coppa ...... Look at the settings, there are pretty easy once you know where they are.

Under "forums and moderators" > edit a forum
There are settings to moderate threads as well as posts. This can be done on a per forum type situation. Some use this for political forums where they want to moderate all the posts in the forum prior to being posted to the public. That way you can look over all the posts and make sure everyone follows the rules in that forum.
Under "usergroup manager" you can also set a usergroup to be moderated.
If you had two usergroups. one group is a trusted regular forum crowd that you can trust to be nice and civil you can allow them to post what information they would like. While another usergroup that may be new members you can watch and make sure they follow the rules and keep spam and vulgar comments out of your discussions.
It is just two ways to moderate traffic for a site owner.
