Paid Subscriptions [people being added that shouldn't]

I don't know why this keeps happening, but it seems to be random. Somehow, some members are being added to the paid subscriptions (premium member on our board), yet they did not make a payment. All of them for some reason have premium as their primary usergroup.

Anyone have any ideas?
