Is it possible to make it so that only your friends can see your profile?

I just reviewed the member.php code and I don't see any option for this. You will need custom code to add this feature. If you have some coding ability then you can post on www.vbulletin.org for help with this.

Here is the group permission for viewing profiles:

Admin CP -> Usergroups -> Usergroup Manager -> Edit Usergroup -> Can View Member Info
