[fixed] Database error... help pleaseeee

I just received this from my vBulletin board through email.

What could the problem be?... Looks like someone was trying to remove a moderator position from someone and it messed up.

That appears to be what happens when you try to remove a moderators that's already been removed. (probably from a double click)

Just needs some more error handling...
