It appears that you're missing a field in your database.
I'm guessing an incomplete upgrade process or something of that nature...
Please run the install/finalupgrade.php script again and be sure to import all the XML's.

This is a known bug. Please run this query to fix it:

UPDATE TABLE phrasetype SET special = 1 WHERE fieldname = 'hvquestion';
