Can they do any damage as guests? Why do they keep showing up
on View Who's Online as viewing the exact same thread, even after they
have been IP banned three times in a row?
We gots us a doozy right now, and I want to make sure they can't do anything.

Anyone can get past the IP ban, they can't do any harm browsing as a guest (Unless they are a hacker and then anything is possible!).

I would suggest having a look on www.vbulletin.org (http://www.vbulletin.org) and trying out these which can give you a little bit of help:

If they show up on the same thread time and time again, this just means the location has been stored remotely and they're revisiting it to try and spam again. If they show up as permission denied on the who's online then it means they are viewing the permission denied page and can't do anything.
