Since making the upgrade, my users are reporting that the insert picture button in the quick reply box isn't working.
I've confirmed this.
When you click on this button, the window shows up and says: "Page not found"
It doesn't seem like this would be effected by any of the mods made to the board...
Any help would be appreciated!

Are you sure you uploaded all files?
The needed one seems to be missing.

What specific file would be missing?
If all of the other buttons in the box work?

Strange because that image exists:

In that case it's from a modification you made. First, reupload all the original vB non-image files (except install.php). Make sure you upload these in ASCII format and overwrite the ones on the server. Also be sure to upload the admincp files to whichever directory you have set in your config.php file. Then run 'Suspect File Versions' in Diagnostics to make sure you have all the original files for your version:
Admin CP -> Maintenance -> Diagnostics -> Suspect File Versions
[Note: In some cases you may also need to remove any of the listed .xml files in the includes/xml directory.]
Next, disable all plugins.
Note: To temporarily disable the plugin system, edit config.php and add this line right under <?php
define('DISABLE_HOOKS', true);
Then if you still have this problem, create a new style and choose no parent style. This will force it to use the default templates. Finally empty your browser cache, close all browser windows then try again. Make sure you change to the new style and view your forums with it. Do you have the same problem?

well, before I went and did all of that, I tested in in FireFox as a tip from a user mentioned.
They didn't have a problem getting the link to work in FireFox, and now, neither have I. Same with IE6.
Users with IE7 are experiencing issues, while users with FireFox and IE6 aren't. This seems to indicate that it's not an issue with a modification. Correct?

I don't have this problem with a default vB and IE7 so I would say that this indicates it's a problem with a modification.
If you really have done everything I suggested in post #8, then fill out a support ticket at:

Please include a complete description of the problem and be sure to include the login info to your Admin CP, phpMyAdmin and FTP in the 'Sensitive Data' field.

When I do this, I get the following error:
Parse error: syntax error, unexpected T_VARIABLE in /home/mp3car/public_html/vbulletin/includes/md5_sums_vbulletin.php on line 2

I've added this to the config.php file that's located in the includes folder. Is this the correct file?

That error means that you have not uploaded the includes/md5_sums_vbulletin.php correctly. Please reupload that file in ASCII format and make sure you overwrite the one on the server.

Due to some other errors we were finding from another issue, I have reuploaded all files and reinstalled.
This has fixed the issue with the insert image button.

I mean upload all the files except install.php and the ones in the images directory.
