At the moment i am running MySQL 4.1.18. I have not upgraded as everything is working fine at the moment.
What are the advantages upgrading to 4.1.22 or 5?
The server is currently running forums in phpBB and also vB and i dont want any of them to brake as its still using php4 because of phpBB.

Upgrading to 4.1.22 is a good idea as it will provide bug fixes over your current version. Upgrading to 5.0.x will add additional features (which you may or may not use) and enhance performance (possibly, it really depends on your configuration) but it may also introduce new bugs or break compatibility with some applications.
If you're not sure whether or not your applications are compatible with MySQL 5.0.x then you would first want to ask the developers of those applications and then second try an upgrade on a test system.

That would certainly be the easier and safer route to take. Some of us like to live dangerously though. :D
Seriously though, vBulletin will run just fine on either. I can't say the same thing for phpBB (as the last time I used it was over a year ago), so it would probably be best, as George said, to just update to 4.1.22 and call it a day.
