I am setting up new servers from scratch and would like to install a very simple control panel, would be great if it was open source as well.
Any suggestions?
I just need it to add new domains, FTP accounts, mail accounts.

I don't see how you add new domains with that? I'm not a reseller I want to create my own domains and assign them IP, what modules they get, etc.

Cpanel rocks :) very simple to use, can perform all the operations mentioned by you easily.

Is WHM open source? It has download links to what be the app but then it says license fee is $450 per year.

WHM/cpanel is commercial paid control panel you can either buy a license or you rent one for a fee between US$15-75/yr via a web host

Are there any open source option or cheaper than buying a commerical license. I don't have a web host. I colocate my servers.

Webmin with a virtualmin module allows you to set up virtual hosting accounts on a web server. You can easily install webmin through SSH
